HOPE ACCESS POINTS (HAPS) 

H.O.P.E. Access Points Program provides information for older veteran adults and individuals with disabilities who need information and assistance about long-term care services, or want to plan for future needs.

Locally, HAPS is a one-stop source of information and assistance for family members, caregivers, disabled adults, and seniors, which helps them navigate through the maze of long-term care services and links them with local and state programs.

Services include:

• Options counseling, 
• Benefits screening, 
• Medicare Part D assistance, 
• Case Management
• Housing Referrals
• Homeless Services and 
• Referrals to other services

Eligibility

H.O.P.E. HAPS program is available to everyone at no cost, regardless of income.
